Audit trail
A chronological, tamper-evident record of who did what, when and with which data or system, including actions taken by automated processes and AI components. It lets an organisation reconstruct how a record changed or a decision was reached.
Why it matters
Regulators, auditors, customers and internal investigators increasingly ask not just what a decision was, but how it was made and who approved it. Without a reliable audit trail, answering that question means piecing together emails and memories.
Where AI assists or automates part of a workflow, the trail should capture the inputs used, the model or rule version, the output, any human review and the final action. That evidence is what makes automated decisions defensible.
In practice
For example, a UK lender using a model to prioritise affordability reviews might log each case’s input data version, model version, score, the reviewer’s decision and any override reason, so any individual decision can be explained months later.
